Endoscopic surgery
Laparoscopic gynaecological surgery allows both visualisation of the pelvic organs and therapeutic procedures to be performed at the same time. As a minimally invasive surgery, it is well accepted and preferred by many gynaecologists and their patients.
What is gynaecological endoscopic surgery?
The gynaecological endoscopic surgery refers to a whole range of techniques that allow for surgically solve gynaecological problems without the need for open or conventional surgery.
This type of surgery has been developed over time with the aim of reducing pain and shortening recovery time, with minimal damage to surrounding tissues.
Gynaecologists trained in minimally invasive gynaecological surgery specialise in assessing and treating women with a wide range of gynaecological conditions including heavy menstrual periods, irregular menstrual periods, pelvic pain, endometriosis and ovarian cysts, among other problems.
The Pérez Bryan Gynaecological Clinic in Malaga has been a pioneer in performing gynaecological laparoscopic surgery, acquiring over many years of experience. Our service is led by Dr. Luis Perez-Bryan Hafnerone of the most prestigious specialists in Endoscopic Surgery in Malagawho performed the first laparoscopic hysterectomy in Andalusia in 1992 and since then has been a national reference as a pioneer in this surgical technique.
The gynaecological endoscopic surgery is probably the most significant advance in surgical practice in this area. It is being performed more and more frequently, due to its well-known advantages: minimal trauma, excellent visualisation, low incidence of complications, reduced adhesions, shorter hospital stays, rapid recovery and good cosmetic outcome.
With technological advances, laparoscopic surgery is rapidly and progressively replacing conventional surgery, except in the most difficult cases. Endoscopy has many applications in modern gynaecology and the list is growing every year.
In general, there are two types of endoscopic surgery in gynaecology: laparoscopy, hysteroscopy
The main indications for laparoscopic surgery are uterine myoma, ovarian tumours, ectopic pregnancy, infertility and endometriosis.
Endoscopic surgical techniques continue to develop very rapidly, helping to meet the diverse needs of patients.

Indications for gynaecological laparoscopic laparoscopic surgery
.01
Adhesion management
Adhesions form as a reaction to the trauma of surgery, infection or endometriosis. In many cases it can be due to tissue manipulation in previous surgeries, blood clots and reaction to suture materials, ischaemia and instruments.
In addition to distorting the anatomy, adhesions can also cause pain, infertility or bowel obstruction. Laparotomy can be performed to remove or cut the adhesions.
The laparoscopic treatment has several advantages, damage to other tissues is minimal, leading to a reduction in the formation of new adhesions. The same is true for bleeding, which is minimal. The laparoscopic surgery has the advantage of reduced postoperative pain, faster recovery and return to normal activities sooner than conventional surgery.
.02
Laparoscopic ovarian surgery
Ovarian cysts are frequent findings during ultrasound. It is important to exclude malignancy by history, examination, ultrasound with Doppler blood flow measurements and tumour markers. If malignancy can be excluded with certainty, then the laparoscopic treatment Atraumatic forceps grasp the cyst wall and ovarian tissue can be treated with blunt dissection, which generates very little damage.
.03
Hysterectomy
The hysterectomy is the removal of the uterus, and can be performed by vaginal, abdominal laparotomy or laparoscopy.
It is a major surgery, with a high level of difficulty, which cannot be performed in all cities or centres. We are fortunate that at Clínica Pérez Bryan all our gynaecologists are trained and qualified to perform hysterectomies by laparoscopy, and have experience in it, even in cases of difficult management due to the large size of the uterus or morbid obesity of the patient (which hinders the entry of the access ports for laparoscopy).
This laparoscopic technique minimises the risk of bleeding, which is also very important when it comes to avoiding blood transfusions during this surgery. The hospital stay will be between 24-48 hours if everything goes well and in a few weeks you will be able to lead a normal life. Unlike when the hysterectomy has to be performed by laparotomy, the hospital stay is a minimum of 3 days and the recovery is much more painful.
.04
Hysteroscopy
The hysteroscopic surgery is performed by direct visual examination of the uterine cavity with an endoscope or hysteroscope. Hysteroscopy has proven invaluable in the evaluation of patients with abnormal uterine bleeding, postmenopausal bleeding, infertility and recurrent pregnancy loss. Not only does it allow for the accurate detection of intrauterine pathology, but it allows the exact location of the pathology and the determination of its extent.
It is also used for patients with intrauterine pathologies such as uterine septa, synechiae, polyps and fibroids. In addition, hysteroscopic resection remains the most appropriate surgical treatment option for submucosal fibroids.
.05
Laparoscopic myomectomy
In the laparoscopic myomectomyOnly the tumours in the uterus are removed, preserving as much of the uterus as possible for future pregnancies.
Our team of gynaecologists can perform laparoscopic myomectomieseven with large or multiple fibroids. If you want to know if you are a candidate for this type of approach, do not hesitate to come to the Pérez Bryan Clinic and we will give you an individual assessment.
