Clinical Pregnancy Monitoring

What is prenatal or pregnancy monitoring?

The clinical management of pregnancy helps to detect possible abnormalities early and reduces the risk of complications during pregnancy, childbirth and the postpartum period.

When to go to the doctor if you are pregnant?

As soon as you suspect that you are pregnant, you need to start the gynaecological monitoring of pregnancy. Your first prenatal appointment will usually be scheduled when you are at least 6 weeks pregnant.

The recommendation of the Spanish Society of Gynaecology and Obstetrics (SEGO) for the correct control and monitoring of pregnancy is that there should be a minimum of eight check-ups during pregnancy: the first before week 10 and then at 12, 20, 26, 26, 30, 34, 36, 38 and 40 weeks.
The frequency of check-ups may vary according to the state of health of the pregnant woman and the foetus.

The Pérez-Bryan Gynaecological Clinic in Málaga offers its patients in its gynaecology unit the best gynaecology clinic in the world. Symptoms of pregnancy

Nausea

They are among the first symptoms early signs of pregnancy. Nausea, which can occur at any time of the day or night, is one of the classic symptoms of pregnancy. For some women, this symptom begins as early as two weeks after conception. Pregnant women also have a very sensitive sense of smell, so various smells can cause nausea in early pregnancy.

Cravings

Like most other pregnancy symptoms, these food preferences can be attributed to hormonal changes, especially in the first trimester when hormonal changes are the most dramatic.

Mood swings

The hormonal change at the beginning of pregnancy can make you unusually emotional, especially in the first trimester.

Fainting and dizziness

As your blood vessels dilate and blood pressure drops, dizziness may occur. In early pregnancy, low blood sugar can also trigger dizziness and fainting.

Elevated basal body temperature

This is the oral temperature when you wake up in the morning. This temperature rises a little after ovulation and remains at that level until your next period. If an elevated basal body temperature is recorded continuously for more than two weeks it may mean that you are pregnant.

Pregnancy Diagnosis

For diagnosis, the following is carried out pregnancy testIt is recommended to do the test a week after the missed period to get the most accurate result. It may give an inaccurate result if the test is taken too early in your cycle.

If you are pregnant, your body needs time to develop detectable levels of chorionic gonadotrophin hormone. This usually takes 7 to 12 days after successful implantation of an egg.

Pregnancy calculator

Most pregnancies last about 40 weeks (or 38 weeks from conception), so the best way to estimate your due date is to count 40 weeks, or 280 days, from the first day of your last menstrual period. Another way to do this is to subtract three months from the first day of your last period and add seven days.

Month-by-month pregnancy monitoring

Each pregnancy consultation and prenatal visit is important for a healthy pregnancy. At the first pregnancy check-up, your doctor will complete a detailed medical history, a complete physical examination, as well as urine and blood tests during this visit.

In the last month, it has even become week-by-week pregnancy monitoring with fetal and contraction monitoring and antenatal care remains very important.

Importance of pregnancy monitoring

According to the SEGO, there are several important objectives to be reached

-01

To optimise the health and quality of life of the pregnant woman and the foetus.

-02

Provide the pregnant woman with all the information about the changes in the different stages of pregnancy.

-03

Babies born to mothers who do not enter a prenatal care programme are three times more likely to have a low birth weight and birth complications. Proper monitoring ensures that the pregnancy is as healthy as possible.

-04

Perform and explain procedures, tests and examinations according to each stage of pregnancy.

-05

Identify the specific risks of each pregnant woman and establish special care in each case.

-06

Early detection and treatment of common disorders during the different stages of pregnancy.

-07

Reduce maternal and foetal morbidity and mortality.

What tests should I do during pregnancy?

Pregnancy tests in the first trimester are:

  • Blood Test
  • Urinalysis
  • Ultrasound for pregnancy monitoringconfirming that the gestational sac is intrauterine, confirming whether the gestation is single or multiple (twins, triplets...).

In a early ultrasoundAt around 9-10 weeks, we can determine quite accurately the date of pregnancy, which sometimes may not coincide with the date of the last menstrual period. We will assess whether the pregnancy is intrauterine, the number of sacs and embryos (to diagnose multiple gestations), as well as evaluate the cardiac activity of the embryo and thus be able to confirm an evolving gestation and rule out a spontaneous miscarriage.

The ultrasound scans allow the follow-up in pregnancy and can be done at various times for various reasons:

Ultrasound scan week 12

In week 12, a slightly more specific ultrasound scan is carried out, in which different parameters such as nuchal translucency are measured, to be combined with the analysis carried out to assess the possibility of chromosomopathies such as Down's syndrome.

Ultrasound scan week 16

Around the 16th week it is already possible to know the sex of the baby, and although all the anatomy is checked, the objective we pursue is more to see a good evolution of the pregnancy, to enjoy the foetal movements and to assess the sex of the baby.

Ultrasound 20 weeks

In week 20, a more specific prenatal diagnostic ultrasound scan is performed, in which possible malformations of the baby are looked for. After this week, the aim of the ultrasound scans is to assess good foetal growth, amniotic fluid, placenta and foetal position.

Management of normal and high-risk pregnancy

Specific factors that may contribute to a eHigh-risk pregnancy include:

Advanced maternal age, particularly over 35 years.

Maternal health problems, such as high blood pressure, obesity, diabetes, epilepsy, thyroid disease, heart or blood disorders, Rh-negative patients, poorly controlled asthma and any type of infection that may increase the risks of pregnancy.

The management of high-risk pregnancy involves a series of special tests, which are indicated according to need and at the discretion of the specialist, for example:

  • Prenatal screening cell-free DNA (cDNA). During this procedure, maternal and foetal DNA is extracted from a maternal blood sample and the foetal DNA is analysed to detect an increased likelihood of specific chromosomal problems. This is what we call Non Invasive Prenatal Testing (NIPT), and although we recommend it in cases of medium-high risk in first trimester screening, it can be performed by any woman who wishes, as it poses no risk to the pregnancy, only a blood test on the mother, and the baby's chromosomes can be obtained.
  • Genetic screening invasive, where amniocentesis or chorionic villus sampling may be recommended. During amniocentesis, a sample of amniotic fluid is removed from the uterus. It is usually performed after the 15th week of pregnancy and can identify certain genetic conditions as well as serious abnormalities of the brain or spinal cord. Chorionic villus sampling, obtained from cells of the placenta, is usually performed between 10 and 12 weeks of pregnancy and can identify certain genetic conditions.
  • Ultrasounds for cervical length assessment
  • Prenatal Doppler ultrasoundwhich is used to check the well-being of the foetus.
